Clinical Pharmacist – Vacancy

Krishna Hospital & Research Centre (KHRC), Haldwani is looking for a Clinical Pharmacist to join our clinical team.

Position Details

- Position: Clinical Pharmacist
- Qualification: Pharm.D (Doctor of Pharmacy)
- Experience: 1–2 years of relevant experience
- Salary: ₹20,000 – ₹30,000 per month
- Location: Haldwani, Uttarakhand
- Eligibility: Candidates from the Kumaon region of Uttarakhand only
- Job Type: Full-time | Permanent
- Work Location: In person

Key Responsibilities

- Work closely with physicians, nurses and other healthcare professionals to ensure safe, effective and appropriate medication therapy.
- Review prescriptions and evaluate the appropriateness, effectiveness, dosage and safety of medications.
- Identify potential drug interactions, adverse drug reactions, medication errors and other medication-related problems.
- Monitor patients' response to medication therapy and provide appropriate recommendations to the treating team.
- Participate in medication reconciliation and ensure accuracy of patients' medication records.
- Provide counselling to patients and attendants regarding medication use, dosage, administration, precautions and possible side effects.
- Support the clinical team in developing and implementing appropriate medication plans.
- Participate in clinical pharmacy interventions and document all interventions appropriately.




- Identify and monitor high-risk medications and patients requiring special attention.
- Conduct regular review of medication usage and support rational use of medicines.
- Coordinate with the Pharmacy and Medical teams regarding medication-related issues.
- Assist in monitoring and reporting adverse drug reactions and drug-related incidents.
- Maintain appropriate clinical pharmacy records and documentation.
- Participate in medication safety, quality improvement and patient safety activities.
- Prepare clinical pharmacy reports and other required documentation.
- Support hospital initiatives related to medication safety and quality of patient care.

Candidate Profile

- Pharm.D qualification is mandatory.
- 1–2 years of relevant Clinical Pharmacy experience preferred.
- Good knowledge of pharmacology, drug interactions, dosage and adverse drug reactions.
- Positive communication and interpersonal skills.
- Ability to work effectively with doctors, nurses, pharmacists and other healthcare professionals.
- Strong documentation and computer skills.
- Candidates must be from the Kumaon region of Uttarakhand.
