Role Overview The Patient Navigator will act as a key link between patients, healthcare providers, and DigiSwasthya's healthcare centres. The role involves assisting patients throughout their healthcare journey, coordinating teleconsultations and referrals, supporting patients in accessing appropriate healthcare services, and ensuring a positive patient experience.

Key Responsibilities

- Assist patients in registering and accessing healthcare services at DigiSwasthya centres and camps.
- Understand patients' basic healthcare requirements and guide them to appropriate healthcare services.
- Facilitate teleconsultations between patients and doctors through the DigiSwasthya platform.
- Coordinate appointments, consultations, follow-ups, and referrals as required.
- Explain the consultation process, medicines, investigations, and follow-up instructions to patients in simple language.
- Maintain accurate patient records, consultation details, referrals, and follow-up information.
- Coordinate with doctors, nurses, pharmacists, and other healthcare team members for smooth patient services.
- Follow up with patients after consultation/referral wherever required.
- Support awareness and outreach activities, health camps,



and community-level healthcare initiatives.
- Assist patients who may have difficulty using digital healthcare services.
- Maintain confidentiality of patient information and follow organizational policies and healthcare protocols.
- Ensure proper documentation and timely reporting of centre activities.
- Support the team in achieving patient consultation, follow-up, and other operational targets.
- Escalate patient concerns, service issues, or emergencies to the appropriate supervisor/medical team.
- Perform other duties related to patient services and healthcare operations as assigned.

Candidate Profile

Education:

- Bachelor's degree in any discipline.
- Candidates with MSW, MA Sociology, Healthcare Management, Public Health, or related qualifications will be preferred.

Experience:

- 1–3 years of experience in patient navigation, healthcare operations, hospital/clinic coordination, community health, telemedicine, counselling, or a related field.
- Freshers with relevant qualifications and strong communication skills may also be considered for suitable locations/projects.

Required Skills
- Good communication and interpersonal skills.
- Ability to communicate effectively with patients from rural and underserved communities.




- Basic understanding of healthcare services and patient-care processes.
- Comfortable using computers, smartphones, and digital healthcare platforms.
- Good documentation and reporting skills.
- Patient-friendly and empathetic approach.
- Ability to coordinate with doctors and healthcare teams.
- Good Hindi and English communication skills; knowledge of the local language will be an advantage.
- Ability to work at healthcare centres and participate in community outreach/health camps.

- Accuracy and completeness of patient documentation.

- Achievement of assigned centre/project targets.
- Participation in outreach and health-awareness activities.

Working Conditions
- The position may require working from DigiSwasthya healthcare centres and participating in health camps/community outreach programmes.
- The candidate may be required to coordinate with patients, doctors, nurses, pharmacists, and field teams.
- Work may involve interaction with patients from rural and underserved communities.

Compensation Salary: 18K-20k pm
