- Manage and support dementia beneficiaries.
- Conduct client visits based on requirements.
- Coordinate with the Head of Dementia.
- Organize and conduct activities for beneficiaries.
- Provide dementia care management under the supervision of a physician or dementia specialist.
- Perform telephonic follow-ups to monitor and implement care plans.
- Attend in-person sessions to address family members' questions regarding resources and care plan implementation.
