Inter-Departmental Liaison: Connect clinical teams, billing departments, pharmacy, and insurance desks to explicit administrative hurdles before discharge.
Patient & Family Communication: Guide patients and families on post-discharge care instructions, follow-up appointments, and medication schedules.
Home Care & Equipment Coordination: Arrange external medical equipment (oxygen cylinders, wheelchairs) or home-nursing support if prescribed by the doctor.
Discharge Summary Tracking: Ensure medical summaries and prescription records are accurately compiled, verified, and signed off by the medical team on time.
Bed Management Support: Minimize delayed discharges to optimize bed availability for incoming emergency or elective admissions.

Pay: ₹18,000.00 - ₹25,000.00 per month
