JOB DESCRIPTION – MRD TECHNICIAN
Department: Medical Records Department (MRD)
Designation: MRD Technician
Reporting To: MRD In-charge / Medical Records Officer / Quality Manager
Employment Type: Full Time
1. Qualification & Experience
- Qualification: Any Degree / Diploma in Medical Records or Health Information Management preferred.
- Experience: Minimum 2 years of experience in a reputed hospital.
- Previous experience in a hospital Medical Records Department is essential.
- Knowledge of hospital documentation, medical terminology and medical record management is preferred.
- Basic computer knowledge and familiarity with Hospital Information Systems (HIS/HMIS) is essential.
2. Job Purpose
To ensure the accurate, complete, secure and timely management of patient medical records, including collection, verification, filing, retrieval, indexing, scanning, storage and maintenance of medical records in accordance with hospital policies and applicable statutory and accreditation requirements.
3. Key Duties & ResponsibilitiesA. Medical Record Management
- Collect and receive patient medical records from various departments.
- Check medical records for completeness, accuracy and proper documentation.
- Arrange and file medical records systematically according to the hospital's approved filing system.
- Maintain both physical and electronic medical records as applicable.
- Ensure records are available to authorized personnel when required.
- Retrieve and issue medical records as per hospital policy.
- Track records issued to departments and ensure timely return.
B. Record Completion & Deficiency
- Review records for missing or incomplete documents.
- Identify documentation deficiencies and coordinate with the concerned departments for completion.
- Maintain records of incomplete medical records.
- Follow up with doctors and concerned departments for pending documentation.
- Ensure records are completed within the prescribed timeline.
C. Coding & Indexing
- Perform basic medical record indexing as per hospital procedures.
- Assist in assigning appropriate disease/procedure classifications where applicable.
- Maintain diagnosis and procedure indexes.
- Ensure accurate patient identification, UHID and admission/discharge details.
- Assist in preparing statistical data from medical records.
D. Discharge Records
- Receive discharged patient files from wards and departments.
- Verify discharge summaries and supporting documents.
- Ensure all required reports and documents are included in the patient file.
- Complete the discharge file assembly and filing process within the prescribed timeframe.
- Maintain records of pending discharge files.
E. Scanning & Electronic Records
- Scan and upload medical documents into the hospital information system as required.
- Ensure scanned documents are clear, complete and correctly linked to the patient's UHID.
- Maintain confidentiality and security of electronic medical records.
- Report system-related issues to the concerned department.
F. Confidentiality & Data Security
- Maintain strict confidentiality of patient medical information.
- Release medical records only to authorized persons according to hospital policy.
- Ensure compliance with hospital policies regarding medical-record access and release.
- Prevent unauthorized access, alteration, loss or damage of medical records.
- Maintain proper documentation of records issued or released.
G. Reports & Statistics
- Maintain daily, weekly and monthly MRD statistics.
- Prepare reports related to admissions, discharges, deaths, outpatient services and other hospital statistics as assigned.
- Maintain records required for hospital audits and accreditation.
- Provide required data to authorized hospital departments and management.
H. Quality & NABH Compliance
- Follow MRD SOPs, policies and work instructions.
- Participate in internal audits and quality-improvement activities.
- Ensure medical records are maintained according to applicable NABH requirements.
- Assist during NABH assessments and medical-record audits.
- Identify documentation gaps and report them to the MRD In-charge.
- Participate in training and competency-development programmes.
I. Record Storage & Preservation
- Maintain proper arrangement and safe storage of physical medical records.
- Ensure records are protected from fire, water, pests, unauthorized access and physical damage.
- Maintain record-retention and archival procedures as per hospital policy and applicable requirements.
- Assist in authorized record disposal after completion of the prescribed retention period.
- Maintain proper documentation of archived and disposed records.
4. Required Skills & Competencies
- Knowledge of medical records management.
- Knowledge of basic medical terminology.
- Positive computer and data-entry skills.
- Knowledge of HIS/HMIS is preferred.
- Good attention to detail and accuracy.
- Good filing and organizational skills.
- Ability to maintain confidentiality.
- Good communication and coordination skills.
- Ability to work independently and as part of a team.
- Ability to handle large volumes of medical records efficiently.
