Role Description This is a full-time, in office Home Health Nurse role based in Chennai. The Home Health Nurse will provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of individualized care plans. Daily responsibilities include monitoring vital signs, administering medications as prescribed, providing wound care, and supporting patients with activities of daily living. The role involves close collaboration with physicians, therapists, and family members to ensure continuity of care, safety, and comfort.



The nurse will also educate patients and caregivers on treatment plans, preventive health practices, and effective home care routines, while maintaining accurate documentation and adherence to clinical protocols.
Qualifications

- Strong foundation in Nursing with current or eligible nursing licensure, and the ability to apply nursing principles in a home health setting.
- Solid understanding of Medicine and clinical protocols to safely administer treatments and coordinate with healthcare professionals.
- Experience in Home Care, including managing care plans, supporting daily living activities, and working independently in patients residences.
- Proven skills in Patient Care, emphasizing empathy, safety, communication, and culturally sensitive, person-centered care.
- Competence in Wound Care, including dressing changes, infection prevention, and monitoring healing progress.
- Diploma or degree in Nursing or a related healthcare field; prior experience in home health or community nursing is an advantage.
- Ability to maintain detailed clinical documentation, follow infection control standards, and adhere to regulatory and ethical guidelines.
- Good communication skills, time management, and the ability to collaborate effectively with multidisciplinary teams and families.
