Role Description The Care Manager role at EVERNEST CARE LLP is a full-time, in office position based in Alappuzha. The Care Manager coordinates and oversees individualized elder-care plans, ensuring that each client’s medical, social, and daily living needs are consistently met.



Day-to-day responsibilities include assessing client requirements, organizing home care services, monitoring health and wellness, scheduling and tracking medical appointments, and facilitating medication reminders. The role involves regular communication with families, documenting care activities, responding to emerging needs, and collaborating with healthcare professionals and community resources. The Care Manager also supports companionship initiatives, promotes safety and dignity at home, and contributes to continuous improvement of care processes and training within the team.
Qualifications
