Position Details

Job Purpose To monitor project operations through the Command Centre, track real-time patient journey activities and alerts, identify exceptions and ensure timely escalation and closure of critical cases.

Key Responsibilities

- Monitor real-time dashboards for patient registrations, appointments, consultations, diagnostics, referrals, admissions, discharges and follow-ups.

- Track patient journey exceptions, delays, missed appointments, pending services and unresolved cases through the system.

- Monitor critical/abnormal diagnostic alerts, emergency cases and hospital referrals and escalate them to the appropriate team.

- Track 48-hour, 7-day and 30-day post-discharge follow-ups and other defined continuity-of-care activities.

- Monitor centre-level operational alerts, service gaps and system exceptions and coordinate with Care Coordinators/Cluster Managers for resolution.

- Maintain live trackers and ensure status updates and case closures are accurately recorded in the system.

- Escalate critical, overdue or unresolved cases within defined timelines and follow up until closure.





- Monitor patient grievances and service-related escalations received through the Command Centre.

- Prepare daily/weekly dashboards and exception reports covering pending cases, alerts, escalations and closure status.

- Analyse recurring alerts and operational trends and highlight key issues to Command Centre/Operations management.

- Coordinate with Medical, Diagnostics, Operations, Quality, IT and other teams for timely resolution of system or patient journey issues.

- Maintain confidentiality of patient information and ensure Command Centre monitoring follows approved processes and access controls.

Qualification & Experience

- Graduate in Healthcare Management, Life Sciences, Nursing, Pharmacy, Operations or relevant field.

- Experience in healthcare operations, command centre, hospital, diagnostics, patient monitoring or MIS preferred.
