Review blood glucose logs
Team-oriented & Community Care
Facilitate group workshops and community education sessions
Act as a liaison between the patient and the broader care team,
Educate fellow healthcare workers
Documentation & Compliance
Maintain meticulous electronic health records (EHR)
Ensure compliance with regulatory standards, patient privacy rights, and hospital safety protocols.
Preferred candidate profile Relevant Experience- 3+ years of clinical experience in direct patient care.
