Project: ABDM-Enabled Care Coordination (AECC)

1. Background The Government of Odisha, through the Department of Health and Family Welfare, will implement the ABDM-Enabled Care Coordination (AECC) initiative in Keonjhar District. Swasti will provide technical assistance to the District Administration and the District Health Society. Keonjhar will serve as the demonstration district for a model that the State intends to replicate.

AECC Aims To Build a Connected, Citizen-centred District Health System Using The Infrastructure Of The Ayushman Bharat Digital Mission (ABDM).

The Initiative Integrates Five Components

Into a Unified Architecture

- Health Information System (HIS): A system implemented across public health facilities.
- Care Coordination Centre (CCC): A platform for patient navigation, referrals, follow-ups, and grievance redressal.
- Red-Amber-Green (RAG) Governance Dashboard: A dashboard supporting district-level monitoring and review.
- Population Health and Wellness Score (PHWS): A health and wellness score published at Gram Panchayat and ward levels.
- ABHA-based Citizen Identity: A digital identity mechanism linking health records across facilities.

The District Administration will own and operate the AECC systems from the outset. All project data must be hosted exclusively on Government-approved infrastructure. Accordingly, the Data and Digital Lead must strengthen district-level capabilities while ensuring effective implementation and sustainable operation of the digital systems.
- Position Purpose

The Data and Digital Lead will serve as the district-level technical anchor for the digital systems and data infrastructure underpinning AECC. The role will ensure that the Health Information System (HIS), Care Coordination Centre (CCC), Red-Amber-Green (RAG) Governance Dashboard, and Population Health and Wellness Score (PHWS) function as an integrated, interoperable, and ABDM-compliant architecture. The role will also ensure that the data generated through these systems are complete, reliable, secure, and effectively used to inform district-level decisions.

The Lead will work closely with the Project Lead and Health Administrator in Keonjhar, act as the primary technical counterpart to technology partners, and support the District Health Society's monitoring and data management functions.

The long-term objective is to enable the district team to independently manage AECC's data and digital functions without external support by the end of the engagement.

- Key Roles and Responsibilities
- Digital Architecture and ABDM Integration
- Serve as the district-level technical counterpart to the HIS and CCC technology partners. Review system configurations, workflows, and integrations against the AECC architecture and interoperability standards.
- Track ABDM readiness across all facilities, including Health Facility Registry (HFR) entries, Healthcare Professionals Registry (HPR) IDs, ABHA linking, and progress against ABDM milestones M1–M3.
- Lead go-live readiness for each implementation phase, beginning with Padampur CHC and progressing towards district-wide coverage. This includes hardware and connectivity checks, user acceptance testing, and sign-off checklists.
- Maintain an issue and change log for each system. Monitor technology partners' responses against agreed service levels and escalate unresolved issues through the Project Lead.
- Provide technical reviews of digital procurements and initiatives in the district,

including kiosks and other District Mineral Foundation (DMF)-funded investments, to ensure integration with AECC and prevent the creation of parallel data systems.
- Coordinate with the State ABDM team, State Programme Management Unit (PMU), and agencies responsible for Government data hosting on integration and hosting requirements.
- ABHA and Citizen Identity Quality
- Monitor ABHA data quality in addition to coverage.

Track

Aadhaar-KYC-verified ABHAs, duplicate records, and active usage at the point of care against cumulative ABHA issuance.

- Support data validation and clean-up exercises, including ABHA linkage with the State family registry wherever approved.
- Work with facility teams and frontline workers to increase ABHA usage during registration and through the scan-and-share workflow.
- Data Quality, Standards, and Governance
- Develop and maintain the AECC data dictionary, indicator reference sheets, and master data covering facilities, healthcare providers, and services.
- Establish and conduct routine data quality checks to assess completeness, timeliness, and consistency across the HIS, CCC, and existing Government reporting systems.
- Support the District Administration in developing and implementing a Data Use Policy covering role-based access, consent, data-sharing protocols, and the requirement to host data exclusively on Government-approved infrastructure.
- Ensure that project practices comply with the Digital Personal Data Protection Act, 2023, and the ABDM Health Data Management Policy.
- RAG Governance Dashboard and PHWS
- Operationalise the RAG Governance Dashboard, including indicator definitions, Red-Amber-Green thresholds, data pipelines, and review formats. Introduce interim paper-based tracking wherever digital data feeds are not yet operational.
- Prepare weekly and monthly RAG reports for the District Collector and other senior district officials, including the Chief District Medical Officer (CDMO) and District Programme Manager (DPM).
- Prepare data packs in the standard reporting format for the fortnightly District AECC Steering Committee meetings.
- Support facility teams in conducting root-cause analyses of Red and Amber alerts and track corrective actions through to closure.
- Support the PHWS methodology and quarterly measurement cycles, including registry-based sampling, score computation, validation, and publication at Gram Panchayat and ward levels.
- Analytics, Evidence, and Learning
- Analyse care pathway data covering referral completion, follow-up adherence, missed appointments, waiting times, and continuity of care. Translate findings into concise, decision-ready briefs.
- Establish baselines and track project indicators against the results framework.

Support funder reporting requirements.

- Ensure that relevant data and documentation are available for independent evaluation and contribute evidence to learning reports, case studies, and the State replication model.
- Capacity Building and Transition
- Train and mentor facility staff, data entry personnel, and the District Health Society's monitoring and data staff on system usage,

data quality management, and dashboard interpretation.
- Prepare standard operating procedures (SOPs), user guides, and data quality playbooks for inclusion in the district's AECC documentation repository.
- Develop and implement a phased transition plan to hand over data and digital functions to Government-designated personnel before project closure.
- Coordination and Reporting
- Work closely with the Project Lead, Health Administrator, CDMO office, and facility in-charges in Keonjhar.
- Submit monthly progress reports to the Project Director covering digital rollout, data quality, and dashboard performance.
- Contribute to quarterly and annual reports for Swasti and the donor.
- Qualifications and Experience
- Educational Qualifications
- A Master's degree in Health Informatics, Public Health, Statistics, Data Science, Computer Applications, or a related field; or a B.E./B.Tech in Computer Science or Information Technology with substantial public health experience.

- Proficiency in data analysis and visualisation tools such as Microsoft Excel, SQL, and Power BI or equivalent platforms.
- Working knowledge of R, Python, or DHIS2 will be an advantage.
- Sound understanding of data quality assurance, data protection, and consent management practices.
- Ability to communicate technical issues clearly to administrators, clinicians, and frontline workers.
- Demonstrated ability to train and mentor personnel with varying levels of digital literacy.
- Solid problem-solving skills and the ability to manage multiple partners, deliverables, and timelines simultaneously.
- Language Requirements
- Proficiency in English is required.
- Working knowledge of Hindi and Odia is preferred.
- Reporting and Performance Management
- The Data and Digital Lead will report to the Project Director, Swasti, and coordinate day to day with the Project Lead, Keonjhar.
- The role will work functionally with the CDMO office and the District Health Society within the governance arrangements established by the District AECC Steering Committee.
- Contract Duration and Extension
- The initial appointment will be for one year.
- The contract may be extended annually up to June 2029, subject to the availability of project funds, satisfactory individual performance, and project requirements.
