Job Summary

We are looking for a detail-oriented Medical Transcriptionist for the Discharge Department with 25 years of experience in a hospital setting. The candidate will be responsible for preparing, transcribing, editing, and proofreading patient discharge summaries and other medical documents, ensuring accuracy, timely completion, and adherence to hospital protocols.

Key Responsibilities

- Transcribe and prepare patient discharge summaries based on doctors notes, dictations, and medical records.
- Ensure accuracy of medical terminology, diagnoses, investigations, treatment details, medications, and follow-up instructions.
- Coordinate with doctors, nursing staff, and other clinical teams to obtain missing information and clarify discrepancies.
- Ensure discharge summaries are completed and processed within the prescribed turnaround time (TAT).
- Proofread and edit medical documents for grammatical accuracy, consistency, and completeness.
- Maintain patient confidentiality and comply with hospital policies and applicable data protection requirements.
- Verify patient demographics, admission details, diagnosis,



and discharge information before finalizing documents.
- Follow up with concerned departments for pending discharge summaries and documentation.
- Maintain proper records of discharge documentation and ensure timely updates in the Hospital Information System (HIS).
- Support the discharge process by ensuring documentation is accurate, complete, and available on time.

Required Qualifications & Skills

- Graduate in any discipline; relevant medical transcription or healthcare documentation training will be an added advantage.
- 2–5 years of relevant experience in a hospital, preferably in the Discharge Department.
- Good knowledge of medical terminology and hospital discharge documentation.
- Strong typing speed with high accuracy and proficiency in English grammar.
- Familiarity with HIS/HMS and MS Office applications.
- Excellent attention to detail, communication, and coordination skills.
- Ability to work under pressure and meet defined timelines in a fast-paced hospital workplace.

Preferred Candidate Profile Candidates with prior experience in preparing discharge summaries, coordinating with consultants, and managing hospital medical records will be preferred.
