- Review and validate medication orders and prescriptions for accuracy and appropriateness.

- Monitor drug therapy for efficacy, safety, adverse drug reactions, and potential drug interactions.

- Identify and report medication errors, adverse drug reactions (ADRs), and drug-related problems.

- Provide clinical pharmacy services, including medication reconciliation and medication history review.

- Participate in ward rounds and collaborate with physicians and nursing staff.

- Provide drug information and evidence-based medication recommendations to healthcare professionals.

- Counsel patients on medication use, dosage, administration, precautions, and possible side effects.

- Monitor high-alert medications, antibiotics,



and other medications requiring special attention.

- Support antimicrobial stewardship and rational drug-use initiatives.

- Maintain accurate clinical pharmacy records and prepare relevant reports.

- Assist in medication safety audits, quality improvement activities, and hospital accreditation requirements.

- Follow hospital policies, clinical protocols, and applicable pharmacy regulations.

EXPERIENCE: Minimum 1 year

Pay: ₹300,000.00 - ₹600,000.00 per year
