Review and validate medication orders and prescriptions for accuracy and appropriateness.
Monitor drug therapy for efficacy, safety, adverse drug reactions, and potential drug interactions.
Identify and report medication errors, adverse drug reactions (ADRs), and drug-related problems.
Provide clinical pharmacy services, including medication reconciliation and medication history review.
Participate in ward rounds and collaborate with physicians and nursing staff.
Provide drug information and evidence-based medication recommendations to healthcare specialists.
Counsel patients on medication use, dosage, administration, precautions, and possible side effects.
Monitor high-alert medications, antibiotics,



and other medications requiring special attention.
Support antimicrobial stewardship and rational drug-use initiatives.
Maintain accurate clinical pharmacy records and prepare relevant reports.
Assist in medication safety audits, quality improvement activities, and hospital accreditation requirements.
Follow hospital policies, clinical protocols, and applicable pharmacy regulations.

EXPERIENCE: Minimum 1 year

Pay: ₹300,000.00 - ₹600,000.00 per year
