Job Summary
We are seeking detail-oriented and experienced Medical Coders specializing in Evaluation and Management (E/M) coding for Inpatient (IP) and Outpatient (OP) services. The candidate will be responsible for reviewing clinical documentation and assigning accurate ICD-10-CM, CPT, and HCPCS codes while ensuring compliance with US healthcare regulations and client guidelines.
Key Responsibilities
- Review and analyze patient medical records for accurate code assignment.
- Assign appropriate E/M, ICD-10-CM, CPT, and HCPCS codes for IP and OP encounters.
- Ensure coding accuracy, completeness, and compliance with payer and regulatory requirements.
- Identify documentation gaps and communicate queries as required.
- Meet productivity and quality targets as defined by the organization.
- Stay updated with coding guidelines, regulatory changes, and industry best practices.
- Participate in internal audits and quality improvement initiatives.
Required Qualifications
- Graduate in any discipline.
- Certification preferred: CPC, CCS, CCS-P, CIC, or equivalent.
- Experience in E/M coding (Inpatient and Outpatient) within the US Healthcare domain.
- Strong knowledge of ICD-10-CM, CPT, HCPCS, and medical terminology.
- Valuable understanding of anatomy, physiology, and disease processes.
- Strong analytical and attention-to-detail skills.
- Proficiency in MS Office and coding tools/software.
Preferred Skills
- Ability to work in a fast-paced environment.
- Excellent communication and comprehension skills.
- Experience with client-specific coding guidelines and quality audits.
Experience
- 4-7 Years of relevant Medical Coding experience in E/M IP & OP.
