Position Summary

KCR Prism Care invites applications from qualified General Nursing and Midwifery (GNM) nurses for patient-care assignments within its home healthcare services.

The position is intended for nurses who can provide safe, responsible, and compassionate nursing care while following prescribed care plans, applicable nursing standards, and organisational procedures.

Candidates should possess relevant nursing qualifications, current nursing registration, sound clinical judgement, and the ability to communicate effectively with patients, family members, and the healthcare team.

Key Responsibilities

- Monitor patients' vital signs, symptoms, and general health status as required by their care plans.
- Provide appropriate nursing care based on authorised medical instructions and individual patient requirements.
- Administer prescribed medications and perform nursing procedures within authorised professional responsibilities and applicable standards.
- Carry out wound care, dressing changes, catheter-related care, and other prescribed nursing interventions when indicated and within scope.
- Identify and promptly report changes in patient condition, abnormal observations, and potential clinical risks.
- Follow designated emergency escalation procedures and seek timely support from the appropriate clinical professional.
- Maintain accurate patient-care notes, vital-sign records, medication documentation, and shift handover records.
- Apply proper infection-control measures, standard precautions, and hygiene practices during patient care.




- Assist patients and family members with relevant care instructions and health education within the nurse's professional scope.
- Coordinate with the assigned nursing supervisor and other healthcare professionals to maintain continuity of patient care.
- Protect patient confidentiality, respect patient dignity, and maintain professional boundaries.
- Follow organisational protocols and ensure that all assigned nursing duties are completed responsibly.

Required Qualifications
- Diploma in General Nursing and Midwifery (GNM) from a recognised nursing institution.
- Valid and current registration with the applicable State Nursing Council.
- Appropriate clinical knowledge and the ability to perform nursing duties safely within the recognised scope of practice.
- Knowledge of vital-sign monitoring, basic nursing procedures, infection control, medication safety, and patient documentation.
- Good communication, observation, teamwork, and patient-care skills.
- Professional attitude, accountability, compassion, and willingness to follow clinical protocols.

Experience Requirements
- Previous experience in a hospital, nursing facility, or home healthcare setting is preferred.
- Candidates with the required qualification and registration may apply according to the experience criteria specified for the actual vacancy.
- Experience in elderly care, post-operative care, long-term patient care, or other relevant clinical assignments is an advantage.
