Key Responsibilities –

1. Medication Review: Review patient medication charts for appropriate drug selection, dosage, frequency, route, and duration of therapy.
2. Drug Interaction Monitoring: Identify and prevent potential drug–drug interactions, drug–food interactions, contraindications, and adverse drug reactions.
3. Clinical Rounds: Participate in clinical rounds with physicians, nurses, and other healthcare professionals to support optimal patient care.
4. Medication Reconciliation: Perform medication reconciliation during patient admission, transfer, and discharge to ensure medication safety and continuity of care.
5. Pharmaceutical Care: Provide patient-specific pharmaceutical care and recommend appropriate medication adjustments based on clinical conditions, laboratory results, and renal or hepatic function.
6. Adverse Drug Reaction Monitoring: Identify, document, report, and monitor adverse drug reactions and medication errors in accordance with hospital policies.
7. Patient Counselling: Educate patients and caregivers regarding medication use, dosage, administration, adherence, precautions, and possible side effects.

Antimicrobial Stewardship: Support rational antibiotic use by reviewing antimicrobial prescriptions, treatment duration, culture reports, and antimicrobial stewardship protocols.
9. Drug Information Services: Provide evidence-based drug information to healthcare professionals and assist in developing and implementing clinical pharmacy protocols and guidelines.
10. Documentation and Quality Improvement: Maintain accurate clinical pharmacy records, medication intervention reports, and patient counselling documentation while supporting medication safety and quality improvement initiatives.

Qualifications and Experience
- Educational Qualification: Doctor of Pharmacy (Pharm.D).
- Minimum Experience: 1 year of hospital experience in clinical pharmacy
- Preferred Skills: Good clinical knowledge, communication skills, analytical ability, teamwork, and proficiency in medical records and hospital information systems.

Pay: ₹300,000.00 - ₹500,000.00 per year
