Functional Responsibilities

· Operations Management

1. You shall report to the Medical Director.

· Clinical Services

1. Review physicians’ medication orders for accuracy, appropriateness, and potential drug interactions.
2. Provide drug information and therapeutic recommendations to doctors and nurses.
3. Monitor patient response to medications and recommend adjustments as needed.
4. Assist in developing individualized patient medication plans in collaboration with the clinical team.
5. Participate in ward rounds, case discussions, and multidisciplinary meetings.

· Patient Counseling & Education

1. Counsel patients and caregivers on proper medication use, dosage, side effects, and storage.
2. Educate patients on chronic disease medication management (e.g., diabetes, hypertension).
3. Provide discharge counseling to ensure continuity of care.

· Drug Monitoring & Safety

1. Conduct therapeutic drug monitoring for narrow therapeutic index drugs (e.g., anticoagulants, antibiotics).
2. Monitor and report adverse drug reactions (ADRs) and medication errors as per NABH guidelines.
3. Participate in pharmacovigilance activities and maintain accurate documentation.

· NABH Compliance & Documentation

1. Maintain all required documentation for medication management in compliance with NABH standards.
2. Participate in Drug & Therapeutics Committee (DTC) activities and contribute to formulary updates.
3. Support antibiotic stewardship and infection control programs.

· Training & Quality Improvement

1.



Conduct educational sessions for nurses, junior pharmacists, and other healthcare staff.
2. Participate in quality improvement initiatives related to medication use.
3. Keep updated with recent drug developments, guidelines, and regulations.

Generic Responsibilities · Job Knowledge: Proficient enough in both the Technical & Functional levels for the given role. Conceptual skills and understanding of Business model are utmost important. Shall know the practices in Industry widely followed from time to time.

· To follow all the rules and regulations of the organization that are prevailing in practice from time to time.

· The Clinical Pharmacist’s role is vital in maintaining a safe, clean, and functional environment, allowing healthcare professionals to focus on delivering high-quality patient care.

· Take ownership of all the duties that you are responsible for at your workplace.

Pay: ₹25,000.00 - ₹35,000.00 per month
