Job Summary

Minimum 1 year of relevant HCC / Risk Adjustment coding

experience.

- Active CPC (Certified Skilled Coder) certification is

mandatory.

- Must provide valid CPC certification details for verification.
- Must not be currently employed / actively working with any

other organization at time of joining.

- Candidates serving notice period may be considered, subject

to company policy.

- Strong knowledge of ICD-10-CM, HCC/Risk Adjustment

concepts, medical terminology, anatomy and clinical

documentation.

- Experience reviewing medical records and identifying

reportable diagnoses preferred.

- Good coding accuracy and attention to detail.
- Ability to meet productivity and quality targets.

KEY RESPONSIBILITIES

- Review medical records and clinical documentation for

HCC/Risk Adjustment coding.





- Identify and accurately assign appropriate ICD-10-CM

diagnosis codes.

- Apply HCC and Risk Adjustment coding guidelines.
- Ensure diagnoses are supported by appropriate clinical

documentation.

- Identify relevant chronic conditions and comorbidities

from patient records.

- Maintain required coding accuracy and productivity

standards.

- Follow client-specific coding guidelines and compliance

requirements.

- Participate in quality audits; address coding errors or

feedback.

- Maintain confidentiality of patient and client information
