Product Owner at Fold Health, owning the care delivery and care management experience of a value-based care platform used by provider groups, MSOs, and ACOs.
About Fold Health
Fold Health is a value-based care technology platform used by provider groups, MSOs, ACOs, and large practices to succeed in value-based care. Fold sits as the intelligence and workflow layer on top of the EHR, integrating deeply across EHRs, claims, HIE feeds, labs, and other data sources to unify data, automate operational work, close care and coding gaps, and give care teams the insight they need to improve outcomes and lower cost. We are building the operating layer that makes value-based care actually workable for the people delivering it.
About the role
This Product Owner owns the care delivery and care management experience of the platform: the workflows care teams, care managers, and coordinators rely on every day. You will shape how care plans, longitudinal patient views, risk assessments, and care and coding gap closure come together into tools that reduce manual work and improve quality and risk accuracy. You will be the bridge between clinical and operational needs and the engineering team that builds for them, turning a complex value-based care problem into software that care teams actually want to use.
We build AI-first, and we want product people who bring that same mindset to how they discover, specify, and ship.
What you will own
- Translate the product vision and clinical and operational needs into explicit, well-defined user stories with complete, testable acceptance criteria, documented in the ticket as the single source of truth.
- Own and maintain a healthy, prioritized backlog so there is always ready, well-defined work ahead of each sprint.
- Act as the voice of the care team in Agile delivery: lead grooming, join sprint planning and daily standups, and unblock requirement questions quickly.
- Deeply understand care management and care coordination workflows:
care plans, longitudinal patient records, risk assessments, transitions of care, chronic and principal care management, annual wellness visits, and care and coding gap closure.
- Work with designers on intuitive workflows for care managers, nurses, and coordinators, providing clear UX requirements while the design team owns the final design.
- Review build output against acceptance criteria before it reaches QA, so quality issues are caught early and not in validation.
- Document the scenarios for user acceptance testing, and own and close the product gaps that surface during UAT.
- Keep product documentation accurate and complete for every release, and own clear, comprehensive release notes.
- Partner with Engineering and QA to prioritize bugs by client and clinical impact and drive them to timely resolution, balancing critical fixes against roadmap progress.
- Define the success metrics for your area, analyze real usage and feedback, and turn that into the next round of improvements.
- Judge whether a client-specific need should be productized into a reusable platform capability or handled as a one-off, so the platform compounds rather than fragments.
- Be the single point of contact for product questions in your area for internal teams, partners, and clients.
- Collaborate with care operations, quality, and risk-adjustment stakeholders to ensure workflows map correctly to value-based care programs and measures.
Who you are
- 2 or more years as a Product Owner, Product Manager, or similar role in a healthcare technology company or a related field.
- An AI-first mindset.
You are already hands-on with AI tools in everyday work, and you want to be on a team that builds product AI-first, applying AI across discovery, specification, and delivery. We are looking for forward thinkers, not AI skeptics.
- Working knowledge of the US healthcare system, primary care delivery, and value-based care models and workflows.
- Familiarity with care management and care coordination: care plans, transitions of care, chronic and principal care management, annual wellness visits, and care gap closure.
- Understanding of Medicare and value-based programs (MA, MSSP, ACO) and how care workflows map to them.
- Exposure to care and coding gaps and risk adjustment, and to the billing that sits behind care management programs such as CCM, TCM, APCM, and AWV.
- Awareness of EHR and HIE systems and integration with them.
- Strong understanding of Agile methodologies, with experience leading cross-functional Agile teams. CSPO or similar certification preferred.
- Clear, structured communication and the ability to drive consensus across clinical, operational, engineering, and client stakeholders.
- Analytical mindset with the ability to define metrics and make data-driven decisions. Ability to write basic SQL to validate healthcare data is a plus.
- Genuine empathy for the care managers, nurses, and coordinators who use the product, and the judgment to design for their day.
Nice to have
- A clinical or care-management background, or direct experience building care management or population health products.
- Familiarity with quality measures and Star Ratings (HEDIS, NCQA, eCQM) and with HCC and RAF risk adjustment.
- Experience with population health, cohorts, and worklist-driven care team workflows.
- Basic understanding of BI and reporting tools and of how features translate into dashboards.
- Awareness of how LLM and AI-enabled workflows can be applied responsibly in healthcare.
