We are seeking skilled and detail-oriented Home Health Medical Coders with experience in Home Health Coding. The ideal candidate should have a sound understanding of ICD-10-CM coding guidelines, OASIS documentation, PDGM requirements, and clinical record interpretation. The selected candidate will be responsible for reviewing home health medical records, assigning accurate diagnosis codes, supporting documentation compliance, and maintaining established productivity and quality standards.

Key Responsibilities

- Review and analyze home health medical records, physician documentation, and supporting clinical notes.

- Assign appropriate ICD-10-CM diagnosis codes based on documented clinical information and applicable coding guidelines.

- Apply coding conventions, sequencing rules, and official guidelines relevant to home health coding.

- Review OASIS documentation and understand its relationship to the home health assessment and coding process.

- Evaluate Plan of Care (POC) documentation for consistency with the available clinical records.

- Apply knowledge of PDGM and its impact on home health reimbursement and coding requirements.

- Identify documentation gaps, conflicting information, and unsupported diagnoses; escalate queries or clarification requests according to established procedures.

- Ensure coding accuracy, completeness, and compliance with client and organizational standards.

- Meet assigned productivity, turnaround time, and quality targets.





- Maintain patient confidentiality and adhere to applicable healthcare data privacy and compliance requirements.

- Participate in quality audits, feedback sessions, and continuous improvement initiatives.

Eligibility Criteria

- Bachelor’s or Master’s degree in Life Sciences, Nursing, or another relevant healthcare-related discipline.

- Prior hands-on experience in US Home Health Medical Coding is required.

- Relevant experience of 1–3 years in Home Health Coding

- Working knowledge of ICD-10-CM coding guidelines and diagnosis sequencing.

- Practical understanding of OASIS documentation, PDGM, and Plan of Care review.

- Ability to interpret clinical documentation and identify coding discrepancies.

- Strong analytical skills, attention to detail, and commitment to coding quality.

- Good communication skills and the ability to work within a team.

- HCS-D certification is preferred and will be an added advantage.
