The practice delivers highly personalized, concierge-level care, coordinating everything from at-home blood draws to specialist visits so families can manage their health in one place over many years. Each family member is treated as an individual patient with their own record and program, supported by a team across therapeutics, diagnostics, genomics, and nutrition. Prvnt uses its own intelligence system to synthesize records and data, allowing physicians to spend more time where human expertise matters. The practice is deliberately limited in size, with capped members per doctor, to maintain privacy, quality, and the standard of care promised. Role Description This is a full-time, on-site Home Care Nurse role based in Chennai. The Home Care Nurse will provide high-quality, individualized nursing care to members in their homes, including routine assessments, monitoring vital signs, administering medications, and supporting treatment, prevention, and longevity programs designed by the medical team. Day-to-day responsibilities include coordinating and performing home visits, managing wound care, ensuring adherence to care plans, documenting clinical findings accurately, and escalating concerns to the physician or Health Partner. The nurse will collaborate closely with doctors,



diagnostics and nutrition teams, and concierge staff to ensure seamless care, arrange tests and follow-ups, and educate members and families on health management and safety at home. The role requires strict adherence to clinical standards, infection control practices, privacy requirements, and a high level of professionalism and empathy in every interaction. Qualifications
• Strong clinical foundation in Nursing and Medicine, with the ability to deliver protected, evidence-based care in home settings.
• Demonstrated expertise in Home Care and Patient Care, including routine assessments, medication administration, and chronic condition management.
• Hands-on experience in Wound Care, including dressing changes, infection prevention, and healing monitoring.
• Valid nursing registration and license, with a recognized nursing qualification (GNM/B.
Sc Nursing or equivalent).
• Experience in home health or primary care settings, preferably with high-touch or concierge-style practices.
• Excellent communication skills in English and local languages, with the ability to build trust with members and families.
• Strong documentation, time-management, and coordination skills, with comfort using digital tools and electronic health records.
• Commitment to privacy, ethical practice, and continuous learning in diagnostics, therapeutics, and preventive care.
