1. Pre-Dialysis Preparation
• Check and record patient pre-dialysis weight, blood pressure, temperature, and pulse.
• Inspect and assess vascular access sites (AV fistula, grafts, or central venous catheters) for patency and signs of infection.
• Prime, calibrate, and set up the hemodialysis machine, blood tubing lines, and dialyzer according to doctor prescriptions and safety checks. 2. During Dialysis Treatment
• Cannulate vascular access or connect the patient safely to the dialysis machine.
• Monitor the patient's vitals, machine parameters, and ultrafiltration rates continuously throughout the session.
• Administer prescribed intra-dialysis medications, heparin, or saline as directed by the nephrologist.
• Identify and rapidly manage intra-dialysis complications such as drops in blood pressure (hypotension), nausea, vomiting, or muscle cramps. 3. Post-Dialysis & Patient Care
• Safely disconnect the patient, apply proper pressure/dressings to access sites, and record post-dialysis weight and vitals.
• Clean, disinfect, and maintain dialysis equipment according to hospital infection-control guidelines.
• Maintain accurate electronic or physical nursing documentation of treatment sheets, fluid removal, and lab results. 4. Patient Education & Counseling
• Educate patients and their families on fluid and dietary restrictions (e.g., sodium, potassium, and phosphorus management).
• Instruct patients on how to care for their vascular access at home and recognize early signs of infection or clotting. Requirements
• Qualification: Diploma in General Nursing and Midwifery (GNM) from a recognized institution.
