Dorika Hospital is a 105-bed multi-speciality hospital located in Tezpur, Assam, established in 2023 to expand access to quality healthcare in Assam and neighbouring north-eastern states in India. Guided by ethical medical practice, patient-centered care, and modern clinical technology, the hospital has treated over 50,000 patients.

Job Location: Tezpur, Assam (On-site, Full-Time)

Eligibility:

- Graduate in any discipline. Qualification in Medical Records Technology, Health Information Management or Hospital Administration will be preferred.
- Minimum 1+ years of experience in medical records management, preferably in a multi-speciality hospital.
- Working knowledge of medical coding (ICD-10) is essential.
- Knowledge of medical terminology, clinical documentation and patient records management.
- Familiarity with HIS/Hospital ERP and MS Office.
- Solid organisational skills, attention to detail and data accuracy.
- Understanding of patient confidentiality and medical records management protocols.

Key Responsibilities:

- Manage and maintain patient medical records, ensuring accuracy, completeness and proper documentation.
- Review patient files for completeness of clinical notes,



investigation reports, discharge summaries and other relevant documents.
- Perform medical coding using applicable ICD-10 classification standards.
- Ensure accuracy and consistency of diagnosis coding and coordinate with clinical teams for documentation clarification, wherever required.
- Manage collection, indexing, filing, retrieval and archiving of medical records.
- Monitor discharge documentation and coordinate with clinical departments for pending records.
- Maintain OPD, IPD, emergency and other patient records.
- Ensure confidentiality, security and controlled access to patient medical information.
- Generate medical records-related reports and maintain necessary registers.
- Support internal audits, NABH documentation and compliance requirements related to medical records.
- Ensure adherence to hospital policies and applicable statutory requirements for medical record retention and management.
