Job SummaryTrevexa Health Solutions is seeking an experienced and detail-oriented Medical Coder II to join our Revenue Cycle Management team. The ideal candidate will have strong knowledge of medical coding guidelines, clinical documentation, and insurance billing requirements to ensure accurate and compliant coding.

Key Responsibilities

- Review medical records and assign accurate ICD-10-CM, CPT, and HCPCS codes based on clinical documentation.
- Ensure coding accuracy and compliance with CMS, payer-specific guidelines, and applicable regulations.
- Perform coding for outpatient and physician services based on assigned specialties.
- Identify documentation deficiencies and coordinate with appropriate teams for clarification.
- Review coding-related claim denials, identify root causes, and recommend corrective actions.
- Conduct coding quality checks and maintain established accuracy and productivity standards.
- Collaborate with billing, charge entry,



and denial management teams to improve first-pass claim acceptance.
- Stay updated on annual coding changes, payer policies, and regulatory requirements.
- Support junior coders by providing guidance and clarification on complex coding scenarios.

Required Qualifications
- Minimum 3 years of hands-on medical coding experience.
- Mandatory: Active CPC (AAPC), CCS (AHIMA), or equivalent recognized medical coding certification.
- Solid proficiency in ICD-10-CM, CPT, HCPCS, and medical terminology.
- Experience in outpatient or professional fee coding.
- Positive understanding of NCCI edits, medical necessity, modifiers, and payer-specific coding guidelines.
- Experience working with EHR/EMR and practice management systems.
- Strong analytical skills, attention to detail, and ability to work independently.

Preferred Qualifications
- Experience with multiple medical specialties.
- Familiarity with Epic, athenahealth, NextGen, or similar EHR systems.
- Experience handling coding audits and coding-related denials.
