POSITION SUMMARY The Care Manager is a field-based qualified responsible for the ongoing wellbeing of clients served by

Tribeca Care. Through regular home visits, the role monitors health parameters, reviews medication compliance and medical records, assesses daily living factors such as mobility, diet and sleep, and provides emotional support. The Care Manager coordinates with caregivers, arranges non-medical needs, keeps family members informed, and documents every visit through the company application.

KEY RESPONSIBILITIES

Client Visits and Health Monitoring

Conduct regular visits to clients across the assigned service areas.

Monitor health parameters during each visit.

¢ Review medication compliance and ensure prescribed routines are followed.

¢ Review medical records and track diagnostic appointments.

Wellbeing Assessment and Support

¢ Assess the client's mobility, diet and sleep patterns.

¢ Provide emotional support to clients.





¢ Arrange and provide support for non-medical needs.

Coordination and Communication

¢ Coordinate with caregivers to ensure consistent, quality care.

¢ Provide regular updates to family members on the client's condition.

Documentation

¢ Maintain accurate visit documentation through the company application.

ELIGIBILITY AND REQUIREMENTS

¢ Educational qualification: Graduate and above.

¢ Age between 25 and 40 years.

¢ Willingness to travel across Kolkata, Rajarhat & Newtown and Howrah.

¢ Comfortable using a mobile application for visit documentation.
