VLMS Global is looking for skilled and detail-oriented HCC Medical Coders to join our healthcare operations team in Pune. The ideal candidate should have a good understanding of HCC coding guidelines, ICD-10-CM coding, and medical record documentation.

Roles & Responsibilities

- Review patient medical records and identify relevant diagnoses for HCC coding.
- Assign accurate ICD-10-CM codes based on clinical documentation.
- Apply CMS-HCC coding guidelines and ensure compliance with coding standards.
- Identify and validate appropriate diagnosis codes based on medical records.
- Review patient charts for supporting clinical documentation.
- Ensure accuracy, productivity, and quality while meeting project targets.
- Identify coding discrepancies and follow established coding guidelines.
- Maintain confidentiality of patient health information (PHI).

Required Skills & Qualifications

- Minimum 1 year of experience in HCC Medical Coding.
- Good knowledge of ICD-10-CM coding guidelines.
- Understanding of CMS-HCC risk adjustment models.
- Knowledge of medical terminology and anatomy.
- CPC / CCS / equivalent medical coding certification preferred.
- Good analytical skills and attention to detail.
- Ability to meet productivity and quality targets.

VLMS Global is seeking experienced and certified HCC Quality Analysts to join our healthcare operations team in Pune. The candidate should have strong knowledge of HCC coding, ICD-10-CM guidelines, medical record auditing, and quality evaluation.

Roles & Responsibilities

- Perform quality audits of HCC medical coding charts.
- Review medical records to ensure accurate diagnosis identification and code assignment.
- Validate ICD-10-CM codes against clinical documentation.
- Ensure compliance with CMS-HCC guidelines and project-specific coding requirements.
- Identify coding errors, discrepancies, and documentation gaps.
- Provide detailed feedback to medical coders to improve coding accuracy.
- Conduct root cause analysis for recurring coding errors.
- Support calibration sessions and process improvement initiatives.
- Prepare and maintain daily, weekly, and monthly quality reports.
- Monitor quality performance and ensure adherence to project quality benchmarks.

Required Skills & Qualifications

- Minimum 1 year of relevant HCC Medical Coding / Quality Auditing experience.
- CPC / CCS / equivalent medical coding certification is mandatory.
- Strong understanding of ICD-10-CM and CMS-HCC guidelines.
- Experience in medical coding quality audits.
- Excellent analytical and auditing skills.
- Good communication and feedback skills.
- Ability to identify coding errors and provide corrective guidance.
