Job Summary The Clinical Reviewer is responsible for reviewing and validating patient medical records to ensure clinical accuracy, completeness, proper documentation, and compliance with healthcare standards. The role involves identifying documentation gaps, reviewing medical coding, ensuring quality standards, and coordinating with healthcare professionals to resolve discrepancies.

- Educational Qualification

- Bachelor's degree in Life Sciences, Allied Health Sciences, or an equivalent healthcare-related discipline.
- Relevant knowledge of clinical documentation and healthcare processes is preferred.

3. Skills and Competencies
- Medical Knowledge: Solid understanding of medical terminology, anatomy, physiology, and clinical documentation.
- Communication Skills: Good written and verbal communication skills for effective coordination and documentation.
- Technical Skills: Proficiency in EHR systems, medical coding software, and MS Office applications.
- Analytical Skills: Ability to evaluate medical records for accuracy, completeness, and compliance.
- Attention to Detail: Ability to identify documentation gaps, inconsistencies, and errors.
- Compliance Knowledge: Familiarity with ICD-10 coding guidelines, HIPAA requirements, and healthcare documentation standards.

4. Roles and Responsibilities
- Clinical Chart Review: Review patient medical records for accuracy, completeness, consistency, and compliance with applicable standards.
- Documentation Quality: Identify missing, incomplete, or inconsistent clinical information and ensure proper documentation.




- Clinical Data Validation: Verify physician notes, patient histories, diagnostic reports, test results, and other relevant medical information.
- Medical Coding Review: Review and validate diagnosis, procedure, and treatment codes, including ICD-10-CM, CPT, and HCPCS, as applicable.
- Coordination and Clarification: Collaborate with healthcare providers, physicians, nurses, and medical coding teams to resolve documentation discrepancies.
- Quality Assurance and Audit Support: Support internal and external audits, identify recurring errors, and contribute to quality improvement initiatives.
- Reporting and Record Maintenance: Prepare review reports, document findings, track corrective actions, and maintain accurate review records.
- Patient Privacy and Confidentiality: Protect sensitive patient information and ensure compliance with HIPAA and applicable data privacy requirements.
