Role Description The Home Health Nurse is a full time, on-site role based in Thane, providing direct clinical care to patients in their homes.

Responsibilities include conducting nursing assessments, administering medications and treatments, managing wound care,



and monitoring vital signs and overall patient condition. The nurse coordinates care plans with physicians and other healthcare professionals, educates patients and families on disease management and home care practices, and ensures adherence to safety and infection-control standards. The role also involves documenting patient progress accurately, responding to changes in health status, and supporting patients and caregivers with compassionate, patient-centered care.

Qualifications

- Candidates should possess strong Nursing skills, including clinical assessment, care planning, and administration of treatments.
- Valid nursing registration/license as per local regulations and completion of an accredited nursing program (GNM, B.Sc Nursing, or equivalent).
