We are looking for a dedicated and compassionate LTP Nurse to support patients undergoing liver transplant evaluation, surgery, and post-transplant care. The candidate will coordinate with multidisciplinary teams, educate patients and families, monitor clinical progress, and ensure seamless continuity of care.
Key Responsibilities
- Coordinate pre-transplant evaluation and patient work-up.
- Assist transplant consultants during patient management.
- Monitor patients before and after liver transplant surgery.
- Provide patient and family education regarding transplant procedures, medications, and follow-up care.
- Coordinate with surgeons, physicians, ICU, OT, laboratory, radiology, and other departments.
- Maintain accurate patient records and transplant documentation.
- Ensure compliance with hospital protocols and infection control practices.
- Schedule follow-up appointments and monitor patient outcomes.
