Job Summary

We are seeking a detail-oriented and experienced Medical Scribe with 2-3 years of experience in US healthcare documentation. The ideal candidate will work closely with Physicians/Doctors and other healthcare providers to accurately document patient encounters in Electronic Health Records (EHR/EMR), ensuring timely and accurate clinical documentation while enabling providers to focus on patient care.

The candidate must possess strong knowledge of medical terminology, anatomy, pharmacology, clinical workflows, and documentation guidelines specific to the US healthcare system.

Key Responsibilities

- Accurately document physician-patient encounters in real time or from recorded consultations.
- Prepare comprehensive and accurate clinical notes including:
- History of Present Illness (HPI)

- Review of Systems (ROS)

- Physical Examination findings

- Assessment and Plan

- Procedures and Orders
- Update patient charts and maintain electronic medical records (EMR/EHR).
- Assist physicians with chart review, medical record retrieval, and documentation completion.
- Document diagnoses, treatment plans, referrals, prescriptions, and follow-up instructions.
- Ensure compliance with HIPAA regulations and patient confidentiality requirements.
- Coordinate with physicians and clinical staff to clarify documentation when needed.
- Review medical records for completeness and accuracy before provider sign-off.




- Support specialty-specific documentation requirements (Family Medicine, Internal Medicine, Cardiology, Orthopedics, etc.).
- Maintain productivity and quality metrics as defined by the organization.

Required Qualifications

- BHMS, BAMS, BDS, or Bachelor's degree in Life Sciences, Nursing, Allied Health, Pharmacy, Biotechnology, or a related healthcare field.
- 2-3 years of experience as a Medical Scribe supporting US healthcare providers.
- Strong understanding of US healthcare documentation standards.
- Proficiency in EMR/EHR systems :

- Excellent knowledge of

- Medical terminology

- Human anatomy and physiology

- Pharmacology

- Laboratory and diagnostic procedures
- Excellent written and verbal communication skills.
- Ability to work independently in a fast-paced environment.

Preferred Qualifications

- Experience working directly with Physicians (MD/DO) in outpatient or inpatient settings.
- Prior experience in specialties such as Internal Medicine, Family Medicine, Cardiology, Orthopedics, Psychiatry, or Emergency Medicine.
- Medical Coding knowledge (ICD-10, CPT, HCPCS) is a plus.
- Experience with AI-assisted documentation tools and virtual scribing platforms.
