Position Summary: The Community Resource Person will be responsible for engaging and mobilizing the local community to actively participate in the health and wellness initiatives and programs offered by respective Health Centre. He/her Primary focus will be to create awareness, build relationships, and encourage community members to adopt healthy lifestyle practices for improved well-being and mobilize them for service uptake at PHC/HWCs.

Key Responsibilities

- ;Coordinate with Health and ICDS ground team for planning the implementation process and assure their participation in project activities.

- ;Organize community meetings and events to raise awareness and gather input from community members.

- ;Mobilize community for PHC service uptake, special outreach sessions for essential primary health care services.

- ;Conduct Community mobilization activities (meetings, mela, celebrating special days, etc.).

- ;Conduct outreach activities to inform community members about the services and programs offered by the health facilities (PHC/HSC)

- ;Collaborate Program Coordinator for the successful implementation of project activities.

- ;Support community health teams like ANMs, ASHAs and AWWs for community members in adopting healthier behaviours.





- ;Foster partnerships and collaborations with local healthcare providers, PRIs, community groups, women groups, SHGs, government agencies, and relevant community-based organizations.

- ;Participate in meetings and forums related to community health and wellness to stay informed about best practices.

- ;Identification and listing of the respondent and collect relevant data and support in community health profiling assessments

- ;Organize and conduct FGDs with targeted respondents

- ;Support in coding and filtering the data

- ;Ready to undertake other assignments, which may be assigned by the supervisor.
