- Participate in ward rounds with physicians to provide pharmacological input on drug therapy.
- Review and monitor patient medication charts for appropriateness, interactions, and dosing accuracy.
- Support the development of individualized pharmacotherapy plans, especially for complex cases.
- Conduct Therapeutic Drug Monitoring (TDM) and interpret pharmacokinetic data.
- Provide clinical advice on adverse drug reactions (ADR) and drugdrug interactions.
- Participate in antimicrobial stewardship and formulary management activities.
