Job Description:
The Patient Care Coordinator is responsible for ensuring a seamless and effective patient experience throughout their healthcare journey. The coordinator will serve as a liaison, ensuring that patients receive the necessary care and support while navigating medical processes.

Key Responsibilities:
Manage patient appointments and schedules to ensure timely access to healthcare services.
Greet and assist patients upon arrival, creating a welcoming environment.
Collect and verify patient information and medical history for accurate record-keeping.
Communicate effectively with healthcare providers and other staff members to address patient needs.
Provide information to patients about treatment plans, procedures, and policies.
Coordinate referrals and follow-up appointments as required.
Handle inquiries and concerns from patients and their families with empathy and professionalism.




Maintain and update patient records, ensuring confidentiality and compliance with healthcare regulations.
Assist in insurance verification and billing processes as needed.
Participate in quality improvement initiatives to enhance patient care services.
