The Nurse Practitioner is responsible for the provision of preventive medical, palliative,



and educational services to promote patient health and wellness and uses standard software and the comprehensive clinical-financial computer system ensuring compliance in accordance with company standards and federal, state, and local standards, guidelines, and regulations.

Essential Activities and Tasks
Care Management - 100%
Conducts comprehensive medical and social histories and performs physical examination of healthy, acutely ill, chronically ill patients or terminal patients.
Treats common acute illnesses, chronic and stable health conditions through prescribing medications and other treatments.
Orders and interprets lab tests and x-rays.
Provides information, instruction, and counseling on health maintenance, health promotion, social problems, illness prevention, illness management, palliative care and medication use.
Refers patients to other health professionals as needed.
Communicates with staff members, patients and families to promote optimal patient outcomes.
Completes documentation accurately and timely while maintaining state, federal and agency regulati
