Role Summary

The Dedicated Care Manager serves as the primary point of contact for Prime members throughout their healthcare journey.

This role is responsible for coordinating healthcare services, conducting regular wellness follow-ups, scheduling consultations, maintaining medical records, and ensuring members receive timely, personalized and seamless healthcare support.

The Care Manager acts as the bridge between members, families, doctors, nutritionists and support teams to deliver premium care experience.

Key Responsibilities

Member Care Management
- Serve as the relationship manager for members.
- Conduct onboarding calls and explain Prime services.
- Perform initial health risk and lifestyle assessments.
- Coordinate for development and maintenance of individualized care plans.
- Conduct scheduled monthly wellness follow-up calls.
- Monitor members with chronic health conditions.
- Identify changes in health status and initiate appropriate interventions.

Care Coordination
- Schedule doctor and nutrition consultations.
- Follow up on post consultations and recommendations.




- Facilitate communication between members, families, and clinicians.
- Ensure continuity of care across all services.

Clinical Support
- Escalate clinical concerns to consulting doctors.
- Assist members in navigating healthcare services.

Documentation & Records
- Maintain with all latest medical records digitally.
- Document every member interaction in the CRM.
- Update care plans and consultation outcomes.
- Ensure confidentiality and compliance with data privacy regulations.

Customer Experience
- Respond to member inquiries via CRM and WhatsApp.
- Resolve service-related issues promptly.
- Manage member escalations professionally.
- Build long-term relationships to improve retention.

Required Qualifications
- B.Sc. Nursing (Mandatory)
- Registered Nurse (State Nursing Council)
- Basic Life Support (Preferred)

Experience
- 1- 3 years in patient care, telehealth, home healthcare,
