Role & responsibilities

- Maintain and organize patient medical records, ensuring all records are complete, accurate, and properly filed.
- Receive, verify, classify, and index medical records received from various hospital departments.
- Maintain both physical and electronic medical records in an organized and easily retrievable manner.
- Enter and update patient information accurately in the Hospital Information System (HIS) / Medical Records System.
- Verify records for missing documents, incomplete information, signatures, and required clinical details.
- Retrieve and provide medical records to authorized departments, doctors, and other personnel as per hospital procedures.
- Maintain proper documentation of record movement, issue, return, and retrieval.
- Assist in the preparation and maintenance of medical records for audits, accreditation, insurance, legal, and administrative requirements.
- Ensure proper confidentiality and security of patient medical information and prevent unauthorized access.
- Coordinate with doctors, nursing, billing,



laboratory, insurance, and other departments to obtain missing or required documents.
- Maintain proper record retention, archiving, and disposal procedures as per hospital policy.
- Assist in medical record quality checks and identify documentation errors or discrepancies.
- Prepare MRD-related reports and MIS as required by the department head.
- Support medical record digitization and scanning activities, ensuring documents are correctly uploaded and indexed.
- Follow hospital policies related to patient confidentiality, documentation, infection control, and data security.
- Assist the MRD/Medical Records Manager in maintaining records required for NABH and other regulatory/accreditation standards.

Qualification - B.Sc MRS / Dip in medical records
