Job Description
The Swasthya Sakhi will serve as a link between the community and the healthcare system, supporting individuals and families in accessing appropriate healthcare services. The role involves community outreach, basic health screening and awareness, facilitating telemedicine consultations, supporting referrals, follow-up of patients, and promoting healthy behaviors.
Key Responsibilities:
1. Community Outreach & Engagement

Conduct regular household and community visits.

Build trust and maintain positive relationships with community members.

Identify individuals and families who may require healthcare support.

Create awareness about available health services.
-
Encourage timely care-seeking and preventive healthcare practices.
2. Basic Health Screening & Identification
-
Support basic health assessments as per the program's approved protocols.
-
Identify common health concerns and potential warning signs.
-
Record relevant patient information accurately.
-
Recognize situations requiring immediate medical attention and escalate appropriately.
3. Telemedicine Support – Arogyapath 2.0
-
Help community members access healthcare through the Arogyapath WhatsApp pathway.
-
Support patient registration and collection of required information.
-
Assist patients in connecting with healthcare providers through audio/video consultations.
-
Ensure patients are comfortable and understand the consultation process.
-
Support communication between the patient and healthcare provider when required.
-
Help patients understand the next steps advised by the healthcare provider.
4. Referral & Follow-up
-
Facilitate referrals to appropriate health facilities when advised.

Explain referral instructions to patients and family members.
-
Follow up with referred patients to understand whether they accessed the recommended care.

Track referral completion and follow-up requirements.

Escalate urgent or high-risk cases immediately according to program protocols.
5. Documentation & Reporting
-
Maintain accurate patient and community records.
-
Update digital systems/applications as required.
-
Record teleconsultation, referral, follow-up, and community outreach activities.

Submit daily/weekly reports to the supervisor.
-
Maintain confidentiality of patient information.
Requirements
Prior Experience:
- 1-5 years of experience in community health, field outreach, healthcare, social work, or patient support is preferred.
- Experience in SHG or community mobilization preferred
- Experience in community/public health programs preferred
Educational Qualification:
- 10th / 12th pass or equivalent.
- ANM, ASHA, GNM, community health worker, or other relevant healthcare/community-health experience will be an advantage.
Skills:
- Good communication and interpersonal skills.
- Ability to communicate effectively with community members.
- Basic understanding of healthcare and community health.
- Basic digital literacy and ability to use a smartphone/WhatsApp/apps.
- Ability to document information accurately.
- Empathy, patience, and respectful behavior.
- Ability to work independently in the community.
- Willingness to travel within the assigned area.
- Ability to maintain confidentiality and Good observation and escalation skills.
