Role Description The Registered Nurse or Medical Doctor Clinical Audit and Remote Care Coordinator is a full-time, in office role based in Sahibzada Ajit Singh Nagar. In this position, the clinician oversees clinical audits of patient charts, care plans, and documentation to ensure accuracy, regulatory compliance, and adherence to clinical standards. The role includes coordinating remote care activities, reviewing telehealth consultations, monitoring patient progress, and collaborating with field nurses and therapists to optimize care.

The coordinator communicates with patients, families, and multidisciplinary team members to clarify care plans, address clinical concerns, and support continuity of care.



Daily responsibilities also include maintaining detailed records, participating in quality improvement initiatives, and providing clinical guidance to staff.

Qualifications

- Clinical expertise: Valid Registered Nurse (RN) license or Medical Doctor (MD) qualification, with strong knowledge of home health, acute care, or primary care practice.
- Audit and quality skills: Experience in clinical chart review, documentation audit, quality assurance, and familiarity with regulatory or accreditation standards for home health care.
- Remote care and coordination skills: Ability to manage telehealth workflows, coordinate care across teams, monitor patient status remotely, and escalate clinical issues appropriately.
- Communication and collaboration skills: Strong written and verbal communication, capacity to educate patients and families, and ability to work effectively with multidisciplinary teams.
- Organizational and technology skills: Proficiency with electronic health records, basic data analysis, time management, and attention to detail in clinical documentation.
- Beneficial qualifications: Prior home health experience, knowledge of care planning and case management, fluency in relevant local languages, and training in evidence-based guidelines or clinical risk management.
