Pay range (depending on experience): $125/hr.

Schedule/Shift: PRN weekdays F2F visits as needed.

Territory/Location: Covering both Portland and Salem areas.

Responsibilities:
- Deliver comprehensive hospice services in accordance with the Nurse Practice Act and under a collaborating physician’s supervision.
- Conduct patient evaluations, health assessments, diagnosis, and management of care plans, including prescribing medications and treatments.
- Perform physical exams, face-to-face assessments, and hospice eligibility evaluations in consultation with the interdisciplinary team.
- Collaborate with patients, families, and healthcare practitioners to coordinate care.
