Role & responsibilities
- Review and validate medication prescriptions for accuracy, dosage, drug interactions, and contraindications.
- Collaborate with physicians, nurses, and other healthcare professionals to optimize patient treatment plans.
- Monitor patient medication therapy and evaluate treatment outcomes.
- Provide drug information and medication counseling to patients and their families.
- Identify, prevent, and resolve medication-related problems and adverse drug reactions.
- Participate in ward rounds and clinical discussions to support evidence-based patient care.
- Ensure protected and effective use of medications according to hospital protocols and clinical guidelines.
- Review patient medical histories, laboratory reports,



and clinical data to recommend appropriate therapies.
- Educate healthcare staff on medication usage, safety practices, and new drug therapies.
- Assist in developing and implementing medication management policies and procedures.
- Monitor antimicrobial usage and support antimicrobial stewardship programs.
- Maintain accurate documentation of clinical interventions and patient medication records.
- Participate in quality improvement initiatives and medication safety audits.
- Ensure compliance with regulatory, accreditation, and hospital standards.
