Experience :
Pharm.D graduate with sound clinical knowledge and positive communication skills.
Freshers are preferred.
Should be willing to work in multidisciplinary patient care, medication safety, antimicrobial stewardship, and clinical documentation activities in a NABH-accredited hospital.
Duties & Responsibilities :
Review Medication Orders For Appropriateness, Interactions, Contraindications, And Dose Adjustments.
Provide Drug Information And Therapeutic Recommendations To Healthcare Specialists.
Support Antimicrobial Stewardship, Including Surgical Antimicrobial Prophylaxis Audits.
Monitor Adrs And Medication Errors.
Document Clinical Interventions.
Assist In Medication Safety, Audits, NABH Compliance,
And Departmental Education & Training.

ADR detection, reporting, and pharmacovigilance activities
