Role & responsibilities
Review and validate medication prescriptions for accuracy, dosage, drug interactions, and contraindications.
Collaborate with physicians, nurses, and other healthcare professionals to optimize patient treatment plans.
Monitor patient medication therapy and evaluate treatment outcomes.
Provide drug information and medication counseling to patients and their families.
Identify, prevent, and resolve medication-related problems and adverse drug reactions.
Participate in ward rounds and clinical discussions to support evidence-based patient care.
Ensure protected and effective use of medications according to hospital protocols and clinical guidelines.
Review patient medical histories, laboratory reports,
and clinical data to recommend appropriate therapies.
Educate healthcare staff on medication usage, safety practices, and recent drug therapies.
Assist in developing and implementing medication management policies and procedures.
Monitor antimicrobial usage and support antimicrobial stewardship programs.
Maintain accurate documentation of clinical interventions and patient medication records.
Participate in quality improvement initiatives and medication safety audits.
Ensure compliance with regulatory, accreditation, and hospital standards.
