Role & responsibilities

- Coordinate all pre-transplant, transplant, and post-transplant activities for heart and lung transplant patients.

- Act as the primary point of contact for transplant patients, families, donors, and multidisciplinary teams.

- Coordinate patient evaluations, investigations, consultations, admissions, and transplant scheduling.

- Maintain transplant waiting lists and ensure compliance with transplant registry and allocation guidelines.

- Coordinate with transplant surgeons, physicians, intensivists, nursing teams, laboratories, blood banks, and diagnostic departments.

- Ensure timely completion of transplant documentation, consent forms, insurance approvals, and regulatory requirements.

- Educate patients and families regarding transplant procedures, medications, follow-up care, lifestyle modifications, and post-transplant precautions.

- Monitor patient follow-up schedules and coordinate post-transplant care and continuity of treatment.

- Maintain accurate patient records, transplant databases, and clinical documentation.

- Coordinate organ retrieval logistics and communication with organ sharing networks and external agencies when required.





- Ensure adherence to hospital policies, transplant protocols, ethical standards, and government regulations.

- Support quality initiatives, audits, accreditation standards, and transplant program evaluations.

- Assist in data collection, reporting, clinical research activities, and transplant outcome analysis.

- Coordinate transplant-related meetings, CMEs, patient counseling sessions, and awareness programs.

- Ensure effective communication and coordination during emergency transplant procedures.

- Monitor and report transplant program performance indicators and operational metrics.

- Maintain confidentiality of patient information and sensitive medical records.

- Perform additional responsibilities assigned by the transplant team or hospital leadership.

Preferred candidate profile

- 13 years of experience in transplant coordination, critical care, cardiology, pulmonology, or clinical coordination within a healthcare setting.

- Experience in heart, lung, or multi-organ transplant programs is preferred.
