Key Responsibilities
Conduct patient assessments and assist in the management of diabetes, obesity, and related metabolic conditions.
Develop and implement individualized treatment plans in collaboration with the supervising physician.
Prescribe and adjust medications as permitted by applicable regulations and clinic protocols.
Provide patient education on nutrition, physical activity, weight management, diabetes self-care, and lifestyle modifications.
Monitor patient progress, review laboratory results, and adjust treatment plans as needed.
Participate in in-person and telehealth consultations.

Care Coordination
Collaborate with physicians, dietitians, diabetes educators, nurses, and other healthcare specialists to ensure comprehensive patient care.
Coordinate referrals, diagnostic testing, and follow-up appointments.
Support patients in achieving treatment goals through ongoing communication and care planning.
Facilitate continuity of care for patients with complex metabolic health needs.





Documentation Skills
Maintain accurate, timely, and comprehensive patient records in the Electronic Medical Record (EMR) system.
Document patient encounters, treatment plans, medication changes, and follow-up recommendations.
Ensure compliance with clinic policies, regulatory requirements, and quality standards.
Review and interpret laboratory and diagnostic results, documenting findings appropriately.
