1. Patient Evaluation & Diagnosis
- Conduct comprehensive geriatric assessments, including physical, cognitive, and functional evaluation.
- Diagnose and manage age-related diseases such as dementia, osteoporosis, hypertension, diabetes, and frailty.
- Assess risk factors including falls, malnutrition, and polypharmacy.
2. Treatment & Care Management
- Develop individualized treatment and care plans for elderly patients.
- Manage chronic diseases and multiple comorbidities.
- Provide palliative and supportive care when required.
3. Multidisciplinary Coordination
- Work closely with physiotherapists, dieticians, psychologists, nurses, and social workers.
- Coordinate rehabilitation and long-term care plans.
4. Medication Management
- Review and optimize medications to avoid polypharmacy and drug interactions.
- Ensure protected prescribing practices for elderly patients.
5. Patient & Family Education
- Educate patients and caregivers on disease management, lifestyle changes, and home care.
- Provide guidance on preventive healthcare and aging-related issues.
6. Documentation & Compliance
- Maintain accurate patient records, assessments, and treatment plans.
- Ensure compliance with hospital policies, NABH standards, and patient safety protocols.
7. Quality & Academic Activities
- Participate in clinical audits, CMEs, and research activities.
- Support quality improvement initiatives in geriatric care.
Qualifications & Experience
- MBBS + MD (General Medicine) / DNB with specialization or experience in geriatrics.
- Fellowship in geriatric medicine preferred.
- Valid State Medical Council / NMC registration.
- Relevant experience in geriatric care preferred.
