1. Check the communication book for any new instructions during every shift
2. Check the assignment Register to know the assignment for the shift
3. Check inventory of all equipment, and instruments as per the inventory register and inform the Team Leader/Nurse Manager in case of any discrepancies noted
4. Inquire and know about the following:
- Department Census
- Admissions in the department
- Transfer in and Transfer out
- Discharge/DAMA
- Death
- Medico- legal Cases
- Details of patients undergoing special investigations/invasive procedures and or Surgery
- Details of patients requiring special considerations (Barrier Nursing Precautions, VIPs, Relatives of Employees etc.)
- Planned discharges during the shift
- Others:
- Issues related to Support Services (Diet/Laundry/Stores/Pharmacy/Maintenance/CSSD/Bio-Medical/Billing/Housekeeping etc)
- Interdepartmental lending /borrowing of equipments or consumables and drugs
1. Take handover of assigned patients and receive new admission/transfer-in to the department
2. Orient the patient and relative to the unit
3. Carry out Nursing Assessment to have baseline data of patients assigned / received
4. Inform necessary details to the Doctor on duty
5. Plan, prioritize, implement and evaluate nursing care
6. Carry out the instructions of the doctor and administer treatment as advised
7. Attend the Consultant's rounds and update changes/special instructions in patient's record
8. Carry out changes / special instructions as advised by the Consultant
9. Assist for procedures as required at the bedside
10. Monitor patients closely as per hospital protocol or as advised by the doctor
11. Complete documentation of assessment findings, investigations done, procedures carried out and the treatment given to the patient during the shift according to hospital protocol
12. Ensure that adequate supply of medicines and consumables are available with the patient
13. Ensure that drugs and consumables in excess or not to be used are returned to the respective department
14. Prepare the patients for surgeries / procedures as indicated
15. Shift the patients to other departments if required as per Hospital Policy
16. Ensure the comfort of the patient and family at all times
17. Ensure replacement of medicines and consumables used from the departmental stock
18. Ensure that billing of all procedures and investigations done during the shift are completed
19. Complete all relevant documents
20. Check the patient’s records for any changes, special orders etc. and update as needed from time to time
21. Check and receive all pending investigations reports, if any
22. Attend to patients needs for activities of daily living as required
23. Maintain ward safety and provide safe environment.
24. Report any special events during the shift to the Team Leader and or Nurse Manager
25. Ensure that all admissions, transfers, Death and discharges are entered in the Admission Discharge Register
26. Ensure that the shift report is completed
27. Endorse the patients to the next duty assigned staff
28. Maintain good interpersonal relationship with all employees, patients, families and community.
29. Ensure that patient's and or family's communications/requirements are attended or communicated to the concerned department/person
30. Ensure that discharged patient's unit is ready to receive current admission/transfer
31. Additional Responsibilities during Night Shift:
- Arrange IP file as per checklist with numbering of sheets
- Keep the folders ready for admission
- Ensure all the procedures performed throughout the day are entered in the bedside procedures
- Indent drugs and consumables in sufficient quantity
- Check the expiry dates of the medicines and send the medicines nearing expiry to the pharmacy
- Prepare/keep ready records to be maintained in the department
1. Communication Skills:
- Ability to comprehend verbal instructions given by the supervisor or head of the department / doctors.
- Ability to comprehend and communicate effectively with patients, relatives, other departments and social and or community agencies
- Ability to comprehend and communicate effectively when a telephonic message is received
- Ability to comprehend written instructions given by the Doctors and the other related departmental personnel / doctors
1. Technical Skills:
- Competent to assess patients on receiving to the Ward
- Competent to handle the following situations in the Ward:
- Admissions
- Transfers- Intra-hospital and inter-facility
- Discharges from the Ward
- Care of the dead and handing over procedures including mortuary protocol
- Medico-legal cases
- Ambulance call protocol
- Assist for Procedures done at the bedside
1. Training Responsibilities:
- Participates in staff educational program
- Help in orientation of new staff
- Participation in planned clinical teaching / SOP Training
- Completes Competency Assessments
- Supervise and guide Junior Staff nurses, Health Care Assistants and Housekeeping staff
- Other Skills:

- Technical skills required for using and maintaining the various equipments and the computer.

- Very quick and prompt in handling patients and providing care

- Competent to identify patients who become critical and to handle emergency situations (e.g. Code Blue)
- Competent to handle different CODES used in the hospital for emergency situations

- Ability to handle & calm an emotionally sad, depressed, anxious patients and their relatives.
- Ability to calm patients who fear OT, procedures, equipment.
