Position Summary The Consultant / Interventional Nephrologist is responsible for providing clinical and procedural renal care across Outpatient (OPD), Inpatient (IPD), Intensive Care Units (ICU/MICU/SICU), and Dialysis/Transplant suites. In a medical teaching hospital setting, the role combines tertiary-level clinical services, protocolized renal replacement therapy (RRT), clinical instruction for undergraduate (MBBS) and postgraduate (MD Medicine) trainees, and strict adherence to National Medical Commission (NMC) and NABH quality benchmarks.
Key Responsibilities
- Clinical Nephrology & Outpatient Services
Conduct daily Outpatient Department (OPD) clinics for acute kidney injury (AKI), chronic kidney disease (CKD stages 1–5), diabetic nephropathy, glomerulonephritis, refractory hypertension, and electrolyte/acid-base disorders.
Provide consultative liaison services for complex multidisciplinary inpatients across Critical Care, Cardiology, Surgical, and Oncology services.
Coordinate long-term preventive nephrology protocols to retard CKD progression in high-risk patient cohorts.
- Dialysis & Extracorporeal Therapies
Direct and clinically supervise the Hemodialysis (HD) unit, prescribing individual dialyzer membranes, blood flows, anticoagulation strategies, and dry weight assessments.
Manage acute continuous renal replacement therapy (CRRT: CVVH, CVVHD, CVVHDF), sustained low-efficiency dialysis (SLED), and hemoperfusion for critically ill ICU patients.
Oversee the Peritoneal Dialysis (PD) program (CAPD / APD), including patient selection, exit-site care, prescription management, and peritonitis protocols.
Supervise water treatment plant (RO system) water chemistry, endotoxin testing,
and infection control compliance in the dialysis unit.
- Interventional Nephrology & Bedside Procedures
Perform real-time ultrasound-guided percutaneous native and graft renal biopsies.
Insert temporary and tunneled cuffed central venous hemodialysis catheters (Internal Jugular, Femoral / Permacath).
Perform bedside placement of peritoneal dialysis catheters (acute rigid/Tenchkoff where applicable).
Coordinate with Vascular/Cardiothoracic Surgery and Urology teams for the planning and maintenance of arteriovenous fistulas (AVF) and AV grafts.
- Renal Transplantation Services
Evaluate prospective living and deceased donor kidney transplant recipients and donors, ensuring adherence to the Transplantation of Human Organs and Tissues Act (THOTA) legal frameworks.
Design and adjust induction and maintenance immunosuppressive regimens based on immunologic risk profiles.
Manage perioperative hemodynamics, post-transplant allograft dysfunction, calcineurin inhibitor (CNI) toxicity, rejection episodes (cellular/antibody-mediated), and opportunistic post-transplant infections.
- Academic & Administrative Governance (Medical College Context)
Deliver structured clinical bedside training, didactic lectures, and case discussions for MBBS students, MD (General Medicine) residents, and dialysis technologist trainees.
Fulfill NMC Teacher Eligibility Qualifications (TEQ)
compliance norms, maintaining biometric AEBAS presence and research documentation.
Participate in departmental clinical audits, morbity/mortality conferences, and clinical trial/research activities.
Ensure department compliance with NABH, NABL (laboratory interfaces), and institutional biomedical waste handling guidelines.
Minimum Qualifications & Eligibility Criteria
Primary Medical Qualification: MBBS from an NMC-recognized institution.
Broad Specialty: MD / DNB in General Medicine or Pediatrics from an NMC-recognized institution.
Super Specialty: DM / DrNB in Nephrology from an NMC-recognized institution/university.
Statutory Registration: Valid, active permanent registration with the National Medical Commission (NMC) or relevant State Medical Council.
Faculty Track Eligibility (NMC Norms):
Assistant Professor / Junior Consultant: Direct appointment upon completion of DM / DrNB in Nephrology.
Associate Professor / Senior Consultant: Minimum 2 to 3 years of post-super-specialty teaching/clinical experience as an Assistant Professor, with required indexed research publications and completion of BCME/BCBR courses as specified by NMC.
Core Competencies & Skills
High proficiency in ultrasound-guided percutaneous renal access and tunnelled central line insertions.
Deep knowledge of advanced immunosuppressive protocols and allograft histology interpretation.
Rapid clinical decision-making during severe electrolyte crises, uremic emergencies, and CRRT trouble-shooting.
Explicit, compassionate counseling capabilities for patients transitioning toward end-stage renal disease (ESRD) and renal replacement modalities.
