Hiring: Physiotherapist – Home Care
Location: Gurugram, Haryana
We are looking for a skilled and patient-focused Physiotherapist to join its Home Care team. The ideal candidate will be responsible for delivering high-quality physiotherapy services, improving patients’ functional independence, and ensuring safe and effective rehabilitation at home.
Key Responsibilities
1. Patient Assessment & Continuum of Care
• Assess patients’ medical history, current condition, and functional status.
• Develop and implement individualized physiotherapy treatment plans.
• Monitor treatment outcomes and modify interventions as required.
• Ensure effective discharge planning and provide home exercise programs and discharge instructions.
• Communicate patient progress and treatment outcomes to consultants and the interdisciplinary team.
2. Clinical Leadership & Team Collaboration
• Participate in departmental meetings, team activities, and continuing education programs.
• Conduct in-service training sessions for staff and physiotherapy assistants.
• Supervise physiotherapy students,
provide clinical guidance, and document their progress.
• Ensure adherence to departmental policies, procedures, and clinical standards.
3. Clinical Documentation & Information Management
• Maintain accurate patient records, clinical notes, registers, and service statistics.
• Ensure timely documentation in the Hospital Information System (HIS), as per organizational policies.
• Maintain and supervise accurate clinical documentation by physiotherapy students.
• Coordinate with consultants and other healthcare professionals regarding patient progress.
4. Patient Safety & Quality Standards
• Follow infection control protocols, protected patient handling practices, and emergency procedures.
• Ensure the safety of patients, staff, and visitors during physiotherapy interventions.
