Job Description
Pranaam Hospitals is looking for a detail-oriented and efficient Discharge Summary Typist to prepare accurate patient discharge summaries and medical documentation in coordination with doctors and other hospital departments.
Key Responsibilities
- Type and prepare discharge summaries based on doctors’ notes, dictation and clinical records.
- Ensure accurate entry of patient details, diagnosis, treatment, procedures, medications and follow-up instructions.
- Proofread summaries for spelling, grammar, formatting and completeness.
- Coordinate with consultants and treating doctors to clarify missing or unclear information.
- Ensure timely completion of discharge summaries to avoid delays in the patient discharge process.
- Update and maintain discharge documentation in the Hospital Information System (HIS/HMS).
- Coordinate with nursing, billing and Medical Records Department (MRD) teams.
- Maintain patient confidentiality and secure medical records.
- Follow hospital documentation policies and applicable NABH requirements.
- Ensure all summaries are submitted for review and approval by the authorized doctor.
Candidate Requirements
- Qualification: Any Degree or Diploma; medical transcription training is an advantage.
- Experience: 1–3 years of experience in discharge summary typing or medical transcription preferred.
- Positive English typing speed and accuracy.
- Knowledge of medical terminology and hospital documentation.
- Proficiency in MS Word and basic computer applications.
- Ability to work under pressure and meet deadlines.
- Good communication, coordination and attention-to-detail skills.
- Willingness to work in hospital shifts as required.
