We are seeking a detail-oriented Remote Form Specialist to join our team. In this role you will complete, review, and submit online forms and documentation for admissions, billing, and compliance teams, ensuring accuracy and confidentiality.

Key responsibilities

- Accurately complete online forms using patient records and internal systems
- Enter data into the electronic health record (EHR), CRM, and insurance portals
- Submit insurance verification, authorization, and benefits-related forms to payers
- Review completed forms for errors and inconsistencies before submission
- Coordinate with admissions, clinical, and billing teams to obtain missing information
- Track form status and maintain organized digital records
- Adhere to HIPAA, 42 CFR Part 2, and company privacy policies

Required qualifications

- High school diploma or equivalent
- 1+ years of experience in data entry, administrative support, or healthcare administration
- Typing speed of 45+ WPM with high accuracy
- Solid attention to detail and a methodical approach
- Proficiency with web‑based tools, PDFs, and spreadsheets (Google Workspace or Microsoft Office)
